How to change to Plan Tracker in a few simple steps

Step 1:

Simply sign up with us and let your existing plan manager know you're changing providers. If you’re not sure what to say, feel free to copy and paste this text into an email for them:

Dear X

This is to advise I would like to cancel my service agreement with you for Plan Management. As per your service agreement, the notice period ends in [x] business days from today’s date.  

When closing the current service bookings in the NDIA portal, please reduce the available funds for all budgets to zero dollars to allow my new Plan Manager access to my funding.  

Please forward any unpaid invoices from my current NDIS Plan to invoices@plantracker.com.au and confirm the date you will release my service bookings.

Step 2:

As soon as your existing provider cancels your current bookings in the NDIA portal, and the available funds for all budgets are set to zero, we can begin your service with us. Most providers have a 15-to-30-day notification period, but you can ask to have this reduced.

What is NDIS plan management?
Including plan management from an NDIS-registered plan manager such as Plan Tracker, means you have support for the admin management of your plan. Plan managers pay your invoices, help manage your budget, choose providers (registered and not registered), and offer guidance.
How do I include plan management in my plan?

Including plan management is simple, and is free to you. In the meeting with your NDIA planner or Local Area Coordinator (LAC) you can request to have your plan ‘plan managed’ and include ‘Improved Life Choices’ in your budget. If you are already on an active plan you can request a soft touch review to add plan management.

Can I change to plan management from self-management?

You can switch from self-management to plan management. You will need to make a plan management request and present it to your NDIA planner or local area coordinator (LAC), and also specify your desired plan management provider.

Can I switch NDIS plan managers?

You can change plan management providers at any time in your plan. To start the process you will need to contact the NDIA on 1800 800 110. You will also need to notify your current plan manager, before your new plan manager can make their service booking.

What is the cost of plan management?
Plan Management is funded for participants who have the ‘Improved Life Choices’ budget category included in their NDIS Plan. You can request your funding to be Plan Managed at your initial planning meeting or plan review meeting with your Local Area Coordinator.
